Responsibilities:
Assists in personal care and daily living activities such as bathing, toileting, grooming, dressing, meal preparation, clothing care and maintenance and general health and safety issues.
Responsible for progress notes and monthly tracking sheets.
Assist with training consumer in all areas that are indicated on consumer's Individualized Support Plans.
In Home Support must reside at the Supported Living site for a minimum of 5 consecutive days of a pay period when the Supported Living site is not the primary residence of staff.
Monitor self-administration of medication as prescribed by the resident's attending doctor.
Provide an atmosphere of family living in a community setting.
Assists or maintains the consumer's home as a clean and safe environment.
This includes heavy household chores such as washing floors, windows, walls, moving heavy items of furniture to make the home safer, etc.
Helps consumers maximize their capacities and bridge the gap between abilities and environmental demands by helping them develop skills, routines, and habits that will allow them as much control as possible over everyday environments.
Perform other functions, as directed, relevant to the position.
Requirements:
Must possess a High School Diploma Minimum of one (1) year experience in working with in a medical, psychiatric, nursing, or childcare setting or working with persons who have a developmental disability.
Must pass a Level II Background Screening.
Must pass a drug test.
Must have transportation, current license, current car registration, current car insurance, and a good driving record.
Must be able to lift 50 lbs.
