Summary:
Under the direct supervision of the Mental Health Supervisor (Centralized Intake Access Unit), this position is the first point of contact for veterans and military families with mental health and or substance use disorder needs , for the Tri County Area (Saginaw, Bay and Midland).
This position is grant funded and dependent on funding availability.
This position helps military veterans and their families understand the array of services available and directs them to the services determined to best meet their need.
This position assures that veterans and their families have access to accurate, understandable, and complete information in order to make choices regarding the services that will best meet their needs.
This position will serve as a liaison between the consumer and various community organizations.
This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
ESSENTIAL DUTIES AND RESPONSIBLITIES:
1.
Functions as a member of the Centralized Access and Intake Unit.
2.
Provides crisis intervention as needed.
3.
Assists in making referrals, coordinating care and providing follow-up.
4.
Complete basic assessment of needs and planning to address those needs.
5.
Act as a liaison between the organizations staff, network providers, stakeholders and the community at large to provide supportive services, crisis intervention, and mental health/substance abuse related education for consumer, families, and significant others as it is related to Veteran Services.
6.
Provides/coordinates training and technical assistance opportunities for the community with any topic surrounding veteran and military families and meeting their needs.
7.
Creates/conducts and/or supports community outreach activities to connect with Veterans within the community.
8.
Engage Veterans in planning for service design and delivery in improving outcomes for VMFs both outside of VA Services and within when the opportunity arises.
9.
Participates in community committees and forums in support of Veterans.
While providing and arranging education, training, and community presentations pertaining to Veteran programing and cultural competency.
10.
Provides and/or coordinates with Veteran's Health Administration (VHA)/Veteran's Benefit Administration (VBA) awareness training to Access Center Staff/CMH Staff and SUD providers within their respective Regions on VA eligibility/benefits/gaps/challenges and best practices.
11.
Coordinate Services and Care with the County Department of Veterans Affairs, State Level Veteran Affairs, and the Federal Level of Veteran Affairs while also serving as a Liaison to behavioral health services in any capacity as it is needed to support the Veteran.
12.
Seeks to identify veterans and military families that would not otherwise be identified for mental health and substance use disorder treatment.
13.
Maintains accurate and complete records in accordance with timeliness and quality requirements, and provides written reports as requested.
14.
Attends staff meetings and/or in-service training sessions.
15.
Provides liaison, consultation, and mental health education service for community agencies and resource persons.
16.
Functions as a member of a person-centered planning team for effective services and supports collaboration.
17.
Collaborates with physicians, social workers, peer supports, and supports to help treat consumers.
18.
Adheres to core values and operating principles of Saginaw County Community Mental Health Authority (SCCMHA) at all times.
19.
Will document within the Electronic Health Record, for both mental health (SENTRI II) and substance use disorders (REMI).
INCIDENTAL DUTIES AND RESPONSIBILITES:
1.
Performs various administrative/clerical functions such a preparing travel vouchers, making copies of documents, filing, time management, data collection, reporting, etc.
2.
Attends meetings, seminars, workshops, and community events related to the public mental health mission.
3.
May present to internal staff and network audiences as well as military groups and their families and other public audiences on VHA/VBA issues, resources and concerns.
4.
May occasionally transport consumers to and from agencies and community resources in personal automobile or agency vehicle.
5.
React productively to change and handles other essential tasks as assigned.
6.
Participates in CQI functions.
7.
Uses computer hardware and software to perform tasks.
8.
Attends and passes mandatory training and keeps their required trainings updated.

Close contact with persons who may not be psychiatrically stable and who may be hostile, aggressive and potentially violent.
May work in shared office space with a fast paced and very stressful environment at times.
May be required to travel between SCCMHA locations to support consumer and families throughout the intake process.
Drives vehicle in all kinds of weather.
Is accessible when out in the field.
Qualifications:
Education:
A Bachelor's Degree is required, preferably in a Human Service field.
Experience:
Must be a US Military Veteran and have knowledge of Veteran Services and needs of military families.
Licenses and Certifications:
Must have a valid Michigan Driver's license with a good driving record.
Knowledge, Skills, and Abilities:
1.
Knowledge of military service and culture.
2.
Knowledge of Veteran Services.
3.
Knowledge of the mental health/SUD services and system of care principles 4.
Ability to interview and communicate effectively with consumers and occasionally deal with irate individuals.
5.
Ability to diplomatically associate and relate to individuals of all social, economic and cultural backgrounds.
6.
Knowledge of and ability to use desktop computer software such as Microsoft Word, Excel , Power Point, virtual telework applications e-mail, etc.
7.
Ability to function independently without immediate access to supervision and to respond to crisis with calm and good judgment.
8.
Ability to maintain appropriate professional boundaries.
